Case Management

Purpose

Upon the completion of an assessment / investigation a case can either close or remain open for ongoing services. If the case remains open, it is referred to as case management.

Definition

Within child protection case management is defined as:

  • A systematic process of ongoing assessment focusing on risk reduction
  • Implementing services / activities, which provide for child safety
  • Coordination of those services / activities
  • Consultation with mental health, schools, medical providers, the county attorney, and the development of a plan to keep children safe after case closure
